Eyelids are one of the most complex structures in the human body. While they enable the eye to be washed during opening and closing and prevent eye dryness, on the other hand it is important in terms of facial appearance. Both the upper and lower eyelids are the fastest aging structures on the human face with advancing age. While on the upper eyelid, sagging is depending on excess skin in general, on the lower eyelid under eye sagging occurs depending on mid face sagging. These two main deformities cause an old and tired appearance. However, it is important to remember that the upper eyelid together with the eyebrow and forehead and the lower eyelid with the middle face tissues should be considered together when planning the intervention to eyelids.

Sagging on the upper eyelid generally causes aging-based bagging (blepharoshalasis). However sometimes patients who are genetically tendency or due to anatomical structure apply us with the complaint that eyelid sagging restricts visual field. While the patient is evaluated, position of the eyebrows, forehead lines, Ptosis causing eyelid sagging, and other conditions should be taken into consideration. In most patients, eyelid sagging can’t be treated solely with eyelid aesthetics and it should be combined with brow suspension and/or botox application.

In general, upper eyelid aesthetics are carried out under local anesthesia and sedation. Necessary interventions are carried out for excess skin basically causing the problem and subcutaneous fat bags, if necessary. The duration of operation varies from patient to patient, but in general it takes approximately 1 hour. Patients can be discharged 2-4 hours after the operation. First control examination is carried out between 3rd and 6th days and sutures on the eyelids are removed. Patients can easily return to their work life approximately one week after operation. Recovery of the post-op swellings takes 1-3 months depending on the patient.

Basic cause of the lower eyelid problem is sagging of the fat pad in the mid face and causing tear-through deformities and sagging in nasolabial area on the edge of the lips. As each patient will undergo a different procedure, surgery decision is taken with detailed examination. To remove this sagging, mid face lift is carried out. Mid face lift is a partial face lift that can be performed with open and closed techniques. With this method, sagging fat pad in the mid face is placed in the right location. Operation is performed under general anesthesia through the eyelid (transconjunctival), eyelid margin (subciliary) or endoscopic. The procedure lasts for approximately 3 hours, and the recovery time is 3-6 weeks depending on the patient’s condition. Another approach is fat Injection procedure which is preferred for younger (younger than 45-year-old) patients having lower eye lid deformities. Fat tissue is obtained from the abdomen of the patient with liposuction procedure and injected to the area where volume loss occurs depending on sagging. This procedure is carried out under general or local anesthesia-sedation and lasts 30 minutes. Eyelid problems are observed frequently and cause tired and old appearance on the face.
